Needs Assessment Surveys
The SHRDSB conducts a survey every three years to identify the needs of residents with PHYSICAL and/or SENSORY disabilities. This survey is used by SHRDSB to develop a Needs Assessment Report, which is compiled every three years and submitted to state and local officials to help determine priorities and funding levels for programs designed to assist people with physical and/or sensory disabilities.
Once again it is time for the Commonwealth of Virginia’s Disability Services Boards (DSB’s) to update their tri-annual Needs Assessment Reports. This is done in accordance with the Code of Virginia section 51.5-48 and the guidelines of the Virginia Disability Council and the Virginia Department of Rehabilitative Services.
The South Hampton Roads Disability Services Board (SHRDSB) is conducting this survey to develop a report to assist the Commonwealth of Virginia and the local cities of Chesapeake, Virginia Beach, Portsmouth, Norfolk, and Suffolk in assessing needs and setting priorities to enhance services to persons with physical and sensory disabilities. Please assist us in this important task.
Please complete the survey if you are a person with a physical or sensory disability. In addition you may complete the survey if you are a friend or family member preparing it on behalf of a person with a disability or if you are associated with an organization that serves persons with disabilities.

The South Hampton Roads Disability Services Board and the Virginia Disability Council are very interested in your opinion.
All responses must be received by December 1, 2008 for inclusion in the 2009 report.
